All patients are responsible for reading and understanding the terms and conditions outlined in our Fees and Cancellations policy. Patients will be notified of the Fees and Cancellation policy at the time of scheduling and will be provided with an electronic copy of the policy via our website and the Updox patient portal. By scheduling an appointment with us, you are expressing your understanding of our Fees and Cancellation policy and will be subject to the outlined fees if you cancel, arrive late, or no-show your appointment. For clarity, a cancellation is defined as any appointment cancelled by the patient less than 24 hours prior to the scheduled appointment time. A late arrival is defined as arriving at the clinic 11-15 minutes after your scheduled appointment time. A no-show is defined as failing to attend a scheduled appointment without any attempts to notify the office staff. If you have any questions or concerns about the above policies, please notify the office staff before you schedule your appointment.
Remember to bring a list of any medications you may currently be taking as well as any previous medical records you may want to provide.
